COPD Education Programme Booking Form

St Pauls Primary Care Network are Offering a new education programme to patients with Chronic Obstructive Pulmonary Disease (COPD).

The Programme consists of four weeks of face-to-face group educational sessions. Upon completing your four-week education programme we will be able to offer you a discounted session at our Fit for life exercise classes which are held in the Old School House Cafe, St Pauls on Wednesdays at 10am. Information about the fit for life classes can be found on our website and facebook page. You will also receive access to the MyCOPD app (details of this are provided in your letter).

The education programme can support you to:

Improve your fitness
Increase your knowledge of your condition
Better manage your condition
Maintain a healthier lifestyle

Patients who are eligible for these education sessions will have recently received a letter with all the necessary information and instructions on how to book your place on the course by calling the number on your letter or by using this form.

Once we have received your submission a member of our team will be in touch to confirm the start date and timings of the programme and will provide any other information that you may need.
