Here are some of the services the PCN offers to help support our practices. These can be broken down into two main categories: Providing additional capacity and Preventative medicine/ Health promotion.

Enhanced access Clinics

St Pauls PCN offers many Enhanced Access Clinic appointments. These clinics include:

GP sessions: Offering appointments with general practitioners for a variety of health concerns

Sexual health contraception sessions: Provide contraceptive options for sexual health

Ear syringing: To clear blocked ears, improve hearing, and address discomfort

Cervical screening: Cervical screening tests for early detection of cervical abnormalities and cancer prevention

Spirometry: Lung function testing, typically used to diagnose and monitor conditions like asthma and COPD

Annual Health Checks: Monitoring the overall health and well-being of patients

Social Prescribing

Our Social Prescribing team aim to connect you to activities, groups, support, and services in and around the local area to support you with emotional, practical, and social problems which may be affecting your wellbeing.

They’re here to listen to you and to put you in touch with the right services to support your needs. Our team try to help you gain more control over your health and find ways to improve your mood and feelings in a way which suits you.

PCN Paramedics

Our PCN Paramedics support the provision of emergency and routine home visits as well as assist GPs, undertake patient assessment, treatment and follow. They often work within the community to deliver personalised patient care to support your individual needs.

Respiratory Clinics

We run a Respiratory Diagnostic Hub that offers spirometry and FeNo testing to support the Surgeries in diagnosis of COPD and asthma.

The PCN also Runs consultant-led multidisciplinary team meetings to offer education and discuss complex cases.

Acute Respiratory Infection (ARI) Clinics

Our ARI clinics are dedicated to managing acute respiratory problems. These clinics are led by a skilled team of GPs, Paramedics, Advanced Nurse Practitioners and Advanced Clinical Practitioners.

ARI clinics provide timely diagnosis and treatment for conditions such as asthma exacerbations, chest infections, and other acute respiratory issues.

Clinical Pharmacists

Our clinical pharmacists work as part of the GP teams to ensure that your prescriptions work best for you. They can work with you directly with you to make sure that you’re on top of your medications and that they are working correctly.

You may speak to our pharmacy team when you need medication reviews, or advice on different medications. There is a range of issues that our clinical pharmacists can help with meaning that you no longer have to see a GP for every issue.

Multidisciplinary Team (MDT) Sessions

St Pauls PCN offer Multidisciplinary team session which bring together various healthcare professionals to provide comprehensive care. 

Here at St Pauls we have:

Care Home MDTs: Ensuring residents receive well-coordinated and holistic care by addressing complex needs through collaborative decision-making.

Respiratory MDTs: This is a consultant led session which supports patients with complex respiratory conditions.

Mental Health Practitioners

St Pauls PCN has two Mental Health Nurses, they provide assessment, diagnosis and treatment appointments and advice and support for gp’s in caring for other patients with mental health issues, improve links with community providers.

Frailty Team

Our dedicated Frailty Team provide proactive, personalised support for people living with frailty to help individuals stay safe and independent at home.

The Frailty Team consists of; a GP with special interest in frailty, a frailty nurse, social prescribing link workers, pharmacists and care coordinators. 

Adults who are living with moderate or severe frailty who are registered with any of the four surgeries within our PCN can use the service. Referrals into the service can be made by your GP or the team may proactively identify you if you have been coded on your record as moderately or severely frail. 

Some of the services the team offer include:

  • Comprehensive Frailty Assessments
  • Medication reviews
  • Personalised care and support plans
  • Respect planning 
  • Social Prescribing
  • Referrals to other services
